After Years of Gut Problems, She Finally Felt Like Herself Again

Written by Dr. Christina Massinople, MD
Board-certified physician, 25 years in medicine. Functional medicine for women, licensed in Alabama, Texas and Florida.
When this woman in her 40s first came to see me, she had been struggling with digestive problems for more than five years.
Her symptoms included persistent diarrhoea, nausea, reflux, fatigue, weight gain, and intermittent anxiety. She described having a “sensitive stomach” for much of her life, but things had become significantly worse beginning in 2013.
She had already tried many of the things women are told to do.
She had done Weight Watchers multiple times. She had worked with a nutritionist and a health coach. She had been diagnosed with SIBO and treated with rifaximin, which provided only limited relief.
And despite all of that effort, she still didn’t feel well and couldn’t seem to lose the weight she had gained.
Her initial Medical Symptoms Questionnaire (MSQ) score was 93, reflecting a significant overall symptom burden.
Looking beyond the current symptoms
Rather than looking at her diarrhoea, reflux, fatigue, anxiety and weight as separate problems, we went back through her history.
There were clues.
She had received multiple courses of antibiotics beginning in childhood, followed by additional antibiotics over the years for acne, urinary tract infections and sinus infections. She had a long history of digestive sensitivity and had eventually developed IBS with diarrhoea and SIBO.
Her history suggested that the gut was an important place for us to start.
So instead of beginning with another weight-loss programme, we focused first on restoring gut health.
The transformation
Her progress didn’t happen overnight, but within the first few months it was clear that things were moving in the right direction.
She lost approximately 15 pounds in those early months, eventually losing about 20 pounds overall.
More importantly, her overall symptom burden began to change dramatically.
Symptom questionnaire score
Lower is better. The Medical Symptoms Questionnaire is a standard measure of overall symptom burden.
Where she started and where she got to
- Weight: approximately 20 pounds lost
- Long-standing digestive symptoms: dramatically improved
- Food tolerance: significantly expanded
The digestive symptoms that had affected her quality of life for years resolved or significantly improved. Her energy improved. She was no longer struggling with the same reflux, diarrhoea and food intolerance that had dictated so much of her daily life.
Eventually, she was able to tolerate a much wider variety of foods again.
In fact, by one of our later visits, she jokingly told me she was doing “too well”.
She felt so much better, and could tolerate so many more foods, that she occasionally found herself drifting away from the habits that had got her there.
That gave us an entirely different conversation to have. Not how do we get you feeling better? but how do we maintain this while still allowing you to live your life?
We talked about an 80/20 approach, enjoying greater flexibility without abandoning the foundations that had helped her feel so well, and using an occasional Reset when she felt she needed to get back on track.
The real win
She originally came to me after years of trying to solve digestive symptoms, fatigue, anxiety and stubborn weight gain.
She had already tried diets. She had already sought nutrition advice. She had already been treated for SIBO, with only partial relief.
We stopped treating weight as the primary problem and started looking at the larger pattern.
Her weight changed, but so did almost everything else.
And perhaps the best measure of success?
She went from structuring her life around what her gut couldn’t tolerate to occasionally needing a reminder not to forget how far she had come.
Individual results vary. This case reflects one patient’s experience and is not a guarantee of similar outcomes.
